    To ensure your child is safe and well supported in school, parents and carers must provide up‑to‑date medical information and complete the relevant forms where required.

    Medical Forms Used in School

    Admissions Form – Medical Information Section

    This form is completed when your child joins the school and is used to tell us about any medical or health needs your child has.
    If any of this information changes, please inform the school as soon as possible via the school enquiries email address.

    Parental Agreement for the Setting to Administer Medicine

    This form must be completed and signed if your child needs medication to be held or given during the school day. It:

    • gives the school permission to administer medication
    • provides clear instructions on dosage and timing

    The school cannot administer medication without this form.

    Medical Care Plan / Asthma Care Plan

    Some children may also need a care plan to ensure staff understand how to support them.

    • The Medical Care Plan is a generic form used for a range of medical conditions. It outlines symptoms, triggers, treatments, medication, emergency procedures, and any equipment required.
    • The Asthma Care Plan is specifically for children with asthma and details inhaler use, asthma triggers, symptoms, and emergency response guidance.

    Printed copies of all medical forms are available from the school office on request. Completed forms can be returned by email or in person.

    Dietary Requirements

    All dietary requirements should be shared during the admissions process. These are managed in partnership with Dolce Catering and can also be updated by parents and carers using the Dolce Catering App.
